Alpine 4-H Horse Camp
July 8 - 11, 2009

Registration Deadline is June 12.  Camp is filled on a first come, first served basis and camp fills quickly.        .

All forms must be mailed to:

● Please fill out the the following forms for each person attending camp:  Camp Registration, Class Registration,   Liability Waiver,  Rules and Camper Health Form.
   
● Each person four years old and older will need to include $54 with their registration.
   
● Incomplete applications will be returned (those missing forms or money).
   
● Please do not send forms by any method that requires a post office pick-up (i.e. Express Mail)
   
● The Information Book is for your use and we recommend you bring it to camp with you.
